As others have mentioned logos should always be designed in vector format for scale ability embroidery screens you never know what the client will do with it latter But I always use a standard letter size canvas horizontal to display the logo or variants for the customer I find it a good size when they call and want a pdf sent over I usually use 500x500 for a standard logo size on Illustrator If its just shapes and letters you can freely resize it without any pixelation Illustrator is what i recommend to use for such task such as logos signs etc

You can really go with the defaults as this does not really impact your artwork directly unless you are also preparing a presentation of some sort in which case a format might be relevant Otherwise start a new file make it Legal or A4 in size and make sure to select CMYK in the Color Mode box There is no precise pixel size a logo should be The size would depend on what size you are reproducing it at which you failed to mention If the logo is a raster image then to get it really quite sharp then you probably need to have it printed at least at 600dpi
